Our Legal Duty
Gentle Green Dental Care LLC is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice. We must follow the privacy practices described in this Notice while it is in effect.
Uses and Disclosures of PHI
We may use and disclose your PHI without your written authorization in the following ways:
- Treatment: To provide, coordinate, or manage your dental care with other health professionals.
- Payment: To obtain reimbursement from your insurance provider or another payer.
- Healthcare Operations: For internal operations such as staff training, quality assessment, audits, or customer service.
Other Uses Permitted or Required by Law
We may also disclose your PHI without your authorization when required by law, such as:

- Organ and tissue donation
- To avert a serious threat to health or safety
Disclosures Requiring Your Authorization
Other uses and disclosures not described above will be made only with your written authorization. You may revoke an authorization in writing at any time.
Your Rights Regarding Your PHI
- Access: You have the right to inspect or obtain a copy of your dental records. In Maryland, we must respond within 21 working days.
- Amendments: You may request that we amend your health records if you believe they are incorrect or incomplete.
- Restrictions: You may request restrictions on how we use or disclose your PHI.
- Confidential Communications: You may request to receive communications in a specific way (e.g., via phone, mail, or email).
- Accounting of Disclosures: You have the right to receive a list of certain disclosures of your PHI made in the past six years.
- Paper or Digital Copy: You may request a paper or email copy of this Notice at any time.
Fees: Under Maryland law, we may charge $0.76 per page, plus postage, and a preparation fee of up to $22.88.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Changes to This Notice
We reserve the right to change our privacy practices and this Notice at any time. Changes will apply to all PHI we maintain, including information previously created or received. An updated Notice will be posted on our website and made available in our office.
